Th is s tudy has 2 aims. a) to fi nd ou t whether thc re are any possibi lities of early influencing malepatte rn Alopecia by topica! therapeutics in the sense of «ha ir maintenance». b) To dcfine easy perfo1·mable methods for its object ive measu remen l. A c linica! s tudy under cont ro lled double blind condit ions was done in 30 male volunteers (age 21-30) wilh mpa type I a nd II (Rook). After a washoul phase of 7 days, 3 test prepa rat ions (placebo and verum al 2 concent rat ions) were daily a pplied lopically a l the scalp for 16 weeks. Ve rum prepara tions contented Trimethylxanthi n (TMX) as an inhibitor of cycl ie AMP phosphodiesterase. Unit a rea tr ichogram con trolls were done before, after 2 weeks and at the end of the study. In add ition to the hair density (hair/cm 2) and the different iation of anagen, dysplast ic and telogen hair bu lbs the suprabulbar d iameter (μm) of a nagen ha irs and the extrae tab le cAMP of a nagen hair bul bs (RIA, NEN Chemicals, pmol/ha ir) were measured. The absolute frequency for each parameler was assessed using con tour ana lysis. The biomeu·ica l compari son between placebo and verum shows a s trong trend in favou r of the verum (TMX O, I%), w hereas an increase of TMX (0,5%) does no t confirm this trend. The best parame ters were the ha ir d ia meter (p =7%), the A/T ratio (p= 15%) and the A+ DIT rat io (p = 15%). These cri teria may help to anticipate therapeu tic efforts before gross visual evidence appears in ha ir maintenance.
